What is a ANA BLOT (ANA Profile)?

Where the ANA screen answers whether autoantibodies are present, the ANA profile answers which ones, and that distinction is what turns a positive screen into a diagnosis. The panel tests for a defined set of antibodies against extractable nuclear antigens, and several of them are strongly associated with particular conditions. Anti-Sm is highly specific for systemic lupus. Anti-Ro and anti-La are associated with Sjogren's syndrome and, importantly in obstetrics, with neonatal lupus and congenital heart block, so their presence in a pregnant woman changes fetal monitoring. Anti-Scl-70 points to diffuse systemic sclerosis, anti-centromere to the limited form, and anti-Jo-1 to inflammatory muscle disease with lung involvement. Anti-RNP in high titre suggests mixed connective tissue disease. The panel narrows a broad autoimmune suspicion to a specific diagnosis with real prognostic consequences. Its limitation is that it should follow a positive ANA and a clinical picture, not precede them: ordered speculatively in a well person it produces isolated weak positives that mean little and cause considerable worry.

Why take this test?

The ANA profile is advised after a positive ANA where a connective tissue disease is clinically suspected, in order to identify which one. The picture prompting it typically includes persistent joint pain and swelling, a photosensitive rash, recurrent mouth ulcers, dry eyes and dry mouth, Raynaud's phenomenon, skin tightening, muscle weakness, unexplained breathlessness suggesting lung involvement, or unexplained kidney abnormality. It is particularly important in pregnancy or pre-conception planning for a woman with known autoimmune disease, because anti-Ro and anti-La status determines whether the fetal heart needs monitoring for congenital heart block. It is also used where a specific condition is suspected on clinical grounds even with a modest ANA. It should be interpreted by a rheumatologist alongside the ANA titre and pattern, inflammatory markers, urine testing and the clinical examination, since individual weak positives in isolation frequently mean nothing.
