What is a Free T4 (FT4)?

Thyroxine, or T4, is the main hormone the thyroid gland releases, and it sets the pace of metabolism in nearly every tissue in the body. Almost all of it travels through the blood bound to carrier proteins, and while bound it is inert. Only the small unbound fraction, Free T4, can enter cells and act. Measuring the free portion therefore reflects what the thyroid is actually delivering, whereas a total T4 rises and falls with carrier-protein levels and can mislead. This distinction matters more than it sounds. Pregnancy, the contraceptive pill, oestrogen therapy, liver disease and serious illness all shift carrier proteins substantially, so a total T4 can look abnormal while thyroid function is entirely normal. Free T4 is interpreted alongside TSH, the pituitary signal that tells the thyroid how hard to work.
