What is a Gram Stain?

The Gram stain is a technique from 1884 that remains the fastest useful answer in bacteriology. The sample is stained, decolourised and counterstained, and bacteria divide into two groups according to how their cell walls hold the dye: Gram-positive organisms retain it and appear purple, Gram-negative organisms lose it and appear pink. Combined with their shape, round cocci or rod-shaped bacilli, and their arrangement in clusters or chains, this narrows the field of possible organisms considerably within about half an hour. That speed is its entire value. A culture takes 24 to 72 hours to grow the organism and test antibiotics against it, and a seriously unwell patient cannot wait, so the Gram stain guides the first empirical antibiotic while the culture is pending. It also reports pus cells, which indicate a genuine inflammatory response rather than harmless colonisation, and it tells the laboratory whether the sample was adequately collected. What it cannot do is name the species or predict antibiotic sensitivity, which is why it is always a companion to culture rather than a substitute.

Why take this test?

A Gram stain is advised on almost any sample where bacterial infection is suspected and speed matters: pus or discharge from a wound or abscess, sputum in suspected pneumonia, a throat or genital swab, urine, and body fluids. It is used to decide whether an infection is likely present at all, since finding pus cells with organisms is very different from a sample showing neither, and to give the treating doctor a rational starting point for antibiotics on the same day. In practice it is nearly always ordered together with a culture and sensitivity from the same sample, and the two reports arrive separately, the stain within hours and the culture after two or three days. Collect the sample before antibiotics are started wherever possible, because even a dose or two can suppress the organisms enough to render both the stain and the culture uninformative.
