What is a HBV DNA?

HBV DNA measures the genetic material of the hepatitis B virus circulating in blood, reported as a viral load in international units per millilitre. Where HBsAg answers whether the virus is present, HBV DNA answers how busy it is, and in chronic hepatitis B that second question drives almost every decision. The infection passes through phases in which the virus may be replicating vigorously while liver enzymes stay normal, or replicating quietly while inflammation is active, and the combination of viral load, ALT and liver assessment is what places a patient in one of those phases. Sustained high viral load is the strongest predictor of progression to cirrhosis and liver cancer over years, which is why it is monitored rather than measured once. It is also the number used to judge antiviral treatment: an effective drug drives the load down and ideally to undetectable, and a load that rises again on treatment raises the question of resistance or of doses being missed. Unlike hepatitis C, chronic hepatitis B is usually controlled rather than cured, so this is a long-term monitoring test.

Why take this test?

HBV DNA is advised for anyone with a confirmed chronic hepatitis B infection, meaning HBsAg that has remained positive beyond six months, in order to determine which phase of infection they are in and whether treatment is warranted. It is monitored at intervals thereafter, typically alongside liver enzymes and periodic liver imaging, because the phase can change over time and a patient not needing treatment today may need it later. It is essential in pregnancy, where a high maternal viral load raises the chance of transmission to the baby and may prompt antiviral treatment in the third trimester alongside the immunisation the newborn receives. It is also checked before immunosuppressive therapy or chemotherapy, which can reactivate a dormant infection dangerously. This is a specialist-directed test; if you have simply had a reactive HBsAg, your doctor will arrange this along with the other markers rather than you ordering it alone.

  • Confirmed chronic hepatitis B, to determine the phase of infection
  • Deciding whether antiviral treatment is needed
  • Regular monitoring alongside liver enzymes
  • Pregnancy, where a high viral load affects transmission risk
  • Before chemotherapy or immunosuppressive treatment
  • Assessing response to antiviral therapy over time

What does a high HBV DNA level mean?

It means the virus is replicating actively, which raises the risk of liver damage over time and may indicate a need for treatment. Your doctor reads it with liver function tests and other markers.

What is the difference between HBsAg and HBV DNA?

HBsAg answers whether the virus is present; HBV DNA answers how actively it is multiplying. HBsAg is the screening test, HBV DNA is the monitoring test that guides treatment decisions in someone already known to be infected.

Does a high viral load mean I have liver damage?

Not by itself. Viral load predicts risk over years rather than measuring damage already present. Liver enzymes, albumin, platelets, imaging and sometimes elastography assess the liver itself, and doctors interpret all of them together.

Is hepatitis B curable like hepatitis C?

Usually not in the same sense. Chronic hepatitis B is generally controlled with long-term antivirals that suppress the virus and protect the liver, rather than eradicated. That is why monitoring continues over years, and why stopping treatment is a decision only a specialist should make.

Why does this matter in pregnancy?

Because a high maternal viral load increases the chance of passing the infection to the baby at birth. Knowing the load lets your doctor consider antiviral treatment in the third trimester, which together with the vaccine and immunoglobulin the newborn receives makes transmission very unlikely.
