What is a Liver Function Test (LFT), without GGT?

A Liver Function Test is a panel, not a single measurement, and its power comes from the pattern across its components rather than any one value. The enzymes SGPT and SGOT leak out when liver cells are inflamed or injured, so they indicate damage. Alkaline phosphatase and GGT rise when bile flow is obstructed, so they point at the ducts rather than the cells. Bilirubin is the pigment the liver clears from worn-out red cells, and when it accumulates it produces jaundice. Total protein and albumin measure what the liver is still manufacturing, which reflects function rather than injury. Reading them together separates conditions that look identical from the outside: an enzyme-dominant pattern suggests hepatitis or fatty liver, an ALP-dominant one suggests a blocked duct or gallstone, and a low albumin with abnormal enzymes suggests the liver has been struggling long enough to lose synthetic capacity. The name is slightly misleading, since most of the panel measures damage rather than function, which is why doctors also consider albumin and clotting when they need to know how well the liver is actually working.

Why take this test?

An LFT is advised for yellowing of the eyes or skin, dark urine, pale stools, itching without a rash, persistent tiredness, nausea, or pain and fullness under the right ribs. It is standard where fatty liver has been seen on ultrasound, in anyone who drinks regularly, and in suspected or known viral hepatitis. It is routine monitoring before and during treatment with medicines that stress the liver, including anti-tuberculosis drugs, statins, methotrexate and long courses of painkillers. It also belongs in the annual review for anyone with obesity, type 2 diabetes or metabolic syndrome, because non-alcoholic fatty liver disease is now common in India and is silent until an enzyme test flags it. It is part of most full-body checkups and of pre-operative assessment.

Do I need to fast for an LFT?

Fasting is not usually required, but follow your doctor's advice if it is combined with a lipid or sugar panel.

What is the difference between an LFT and just testing SGPT?

SGPT is one biomarker inside the panel. On its own it tells you the liver is irritated but not why. The LFT adds SGOT, bilirubin, alkaline phosphatase, protein and albumin, and it is the pattern across those that separates an inflamed liver from a blocked bile duct from a failing one.

My LFT is abnormal. Does that mean liver disease?

Not necessarily. Mild enzyme elevations are common and frequently come from fatty liver, a recent medicine, alcohol, or even strenuous exercise in the preceding days. Your doctor interprets the pattern and your history, and will often simply repeat the panel after an interval before investigating further.

Can my LFT be normal and my liver still be damaged?

Yes. In advanced cirrhosis there may be too few functioning cells left to leak enzymes, so the transaminases can read normal or even low while the liver is badly scarred. This is why albumin, bilirubin and clotting matter, and why imaging is sometimes needed alongside.

This panel is without GGT. Does that matter?

GGT is a bile-duct enzyme that mainly helps confirm that a raised alkaline phosphatase is coming from the liver rather than bone, and it is sensitive to alcohol. For routine screening this panel is sufficient; if your ALP is raised or alcohol-related liver disease is a specific question, ask your doctor whether to add GGT.
