It happens when the ovaries produce more androgens than usual, disrupting the normal cycle of egg release. The good news is that a small set of blood tests can give a clear picture of what is happening, and most women manage PCOS very well once they know what they are dealing with.

Which test tells you what

The LH to FSH ratio is a key marker in PCOS - when LH is significantly higher than FSH it suggests the ovaries are being driven to produce more androgens rather than releasing eggs normally. Prolactin is checked to rule out another hormonal condition that can mimic PCOS, since high prolactin causes very similar cycle disruptions. Total testosterone helps confirm androgen excess, which explains acne, facial hair, and scalp thinning. Fasting blood sugar reveals insulin resistance, which is present in many women with PCOS and makes both weight gain and long-term health risks worse.

When to get tested

Get tested if you have had irregular or skipped periods for three months or more, especially if you also notice acne, facial hair, or unexplained weight change. Women trying to conceive without success after six months of trying should not wait - early testing can identify a hormonal cause quickly. Testing is also sensible if a close female relative has been diagnosed with PCOS, since the condition runs in families. You do not need a referral to book these tests; a fasting sample in the morning gives the most accurate results.
