What is a APLA (Anti Phospholipid Antibody) IgM?

This measures the IgM class of anticardiolipin antibodies, one of the three laboratory criteria for antiphospholipid syndrome, an autoimmune condition in which antibodies against phospholipid-binding proteins paradoxically make the blood more likely to clot. IgM is generally the first antibody class to appear in an immune response, and in this setting it carries somewhat less weight than IgG: isolated IgM positivity, particularly at low titre, is more often transient and more often follows an infection, so it is the less specific of the two classes. That does not make it unimportant, because some patients are positive for IgM alone, and missing them by testing only IgG would leave a treatable cause of clotting or pregnancy loss undiagnosed. The rules that govern the whole panel apply here without exception. A single positive result establishes nothing: antibodies must be persistently present on a repeat sample at least twelve weeks apart, and there must be a clinical event, either confirmed thrombosis or a defined pattern of pregnancy loss, before the syndrome is diagnosed.

Why take this test?

IgM anticardiolipin is measured as part of the antiphospholipid panel, alongside IgG, lupus anticoagulant and anti-beta-2 glycoprotein I, wherever that syndrome is being considered. The clinical situations prompting it are an unexplained blood clot, particularly a deep vein thrombosis, pulmonary embolism or stroke in a younger person or at an unusual site; recurrent pregnancy loss meeting the defined criteria; lupus, where these antibodies commonly coexist; and an unexpectedly prolonged clotting test in someone without a bleeding tendency. It is not ordered in isolation, because the different tests identify overlapping but not identical groups of patients and testing only one can miss the syndrome. Do not act on a single positive result: the twelve-week repeat is what distinguishes a transient post-infectious antibody from genuine disease, and that distinction determines whether someone takes anticoagulants for life.
