What is a Beta 2 Glycoprotein (IgG & IgM)?

Beta-2 glycoprotein I is the protein that antiphospholipid antibodies actually target. For years the standard test measured anticardiolipin antibodies, but it emerged that the clinically significant ones bind cardiolipin only in the presence of this protein, and that antibodies directed at beta-2 glycoprotein I itself correlate better with the events that matter, namely thrombosis and pregnancy loss. That makes this the more specific of the two solid-phase tests: it produces fewer of the transient positives that follow infections, and a positive result carries more weight, particularly at high titre and particularly in the IgG class. It is one of the three laboratory criteria for antiphospholipid syndrome, alongside lupus anticoagulant and anticardiolipin. As with the others, a single positive result establishes nothing. Confirmation requires persistence on a repeat sample at least twelve weeks later, together with a clinical event. Patients positive for all three tests, sometimes described as triple positive, carry the highest risk of recurrent thrombosis and are managed most aggressively.

Why take this test?

This test is advised as part of the antiphospholipid panel wherever that syndrome is being considered: an unexplained blood clot, especially in a younger person or in an unusual site, an arterial event such as a stroke without conventional risk factors, or recurrent pregnancy loss meeting the defined criteria. It is particularly valuable where anticardiolipin has come back weakly or equivocally positive, since its greater specificity helps separate genuine disease from a post-infectious blip. It is also part of the assessment in lupus, where these antibodies commonly coexist and change how the patient is managed. Because the three tests identify overlapping but not identical groups of patients, the panel is ordered as a whole rather than selectively. Any positive result needs the twelve-week repeat before conclusions are drawn, and interpretation belongs with a rheumatologist or haematologist.
