What is a CA 15.3?

CA 15-3 is a protein shed by some breast cancer cells into the bloodstream, and its role is narrow, specific and frequently misunderstood. It is a monitoring tool for a cancer already diagnosed, not a way of finding one. It is not sensitive enough to screen: many early breast cancers produce little or none, so a normal result offers no reassurance whatsoever. It is not specific enough to diagnose: benign breast disease, liver disease, pregnancy, endometriosis and several other conditions raise it. Ordering it out of anxiety therefore tends to generate either false comfort or a frightening number with an innocent explanation, and neither serves the person who asked for it. Where it genuinely earns its place is over time in someone under treatment or follow-up for advanced or metastatic breast cancer. There, serial measurements carry information: a falling level supports that treatment is working, a rising trend across successive samples can signal progression or recurrence, sometimes before imaging shows it. What matters is the trajectory across several readings, not any single value, and a modest fluctuation between two tests is often meaningless.
