What is a CA 19.9?

CA 19-9 is a carbohydrate antigen produced by cells of the pancreas and biliary tract, and it is most associated with pancreatic cancer, though it also rises in cancers of the bile duct, gallbladder, stomach and colon. Like other tumour markers, its usefulness lies almost entirely in monitoring rather than detection, and two specific quirks make that especially true here. The first is that anything obstructing bile flow raises it, often dramatically: a gallstone in the common bile duct, cholangitis or any cause of jaundice can produce a very high CA 19-9 with no cancer at all, and the level falls once the obstruction is relieved. Pancreatitis, cirrhosis and even some benign lung and thyroid conditions also raise it. The second is genetic. Around five to ten percent of people lack the Lewis blood group antigen needed to produce CA 19-9 at all, and in those individuals the level stays essentially zero even with advanced pancreatic cancer, so the test is uninformative for them. Together these mean neither a high nor a low value can be taken at face value.

Why take this test?

CA 19-9 is ordered by a specialist in the context of a suspected or diagnosed pancreatic or biliary cancer, not as a screening test. Its recognised roles are establishing a baseline before treatment, supporting assessment of how extensive a known cancer is, monitoring response to chemotherapy or surgery, and surveillance for recurrence afterwards, where a rising trend across serial samples can precede radiological evidence. It is sometimes measured when imaging has already shown a pancreatic mass, to add information rather than to make the diagnosis, which requires tissue. It should not be added to a routine health panel, and it is not the right response to abdominal pain or weight loss. Those symptoms need clinical assessment and imaging. If your level is raised, discuss it with the doctor who ordered it, because obstruction of the bile duct is a far commoner explanation than cancer.
