What is a Calcium?

Almost all the calcium in your body, around 99 percent, is locked into bone. The small fraction circulating in blood does something quite different: it lets muscles contract, nerves fire, blood clot and the heart beat in rhythm. Because those functions cannot tolerate variation, the body defends the blood level tightly, using parathyroid hormone and vitamin D to pull calcium out of bone or absorb more from the gut whenever it dips. That defence is why the test is informative in an unexpected way. A blood calcium that has drifted out of range does not usually mean you are eating too little or too much calcium; it means the control system itself is disturbed, most often by the parathyroid glands, by kidney disease, by vitamin D deficiency, or occasionally by a cancer affecting bone. It also explains why a normal blood calcium does not rule out poor bone health or osteoporosis: bone can be steadily depleted to keep the blood level perfect. One practical point is that roughly half of blood calcium travels bound to albumin, so a low albumin lowers total calcium without any true deficiency, which is why doctors interpret the two together.

Why take this test?

A calcium test is advised for bone or muscle pain, frequent muscle cramps or twitching, tingling around the mouth or in the fingers, unexplained fatigue, excessive thirst with frequent urination, constipation, or confusion, all of which can accompany a calcium that is too low or too high. It is standard in the assessment of recurrent kidney stones, chronic kidney disease and suspected parathyroid disorders, and it is routinely paired with vitamin D when bone health is the question, since vitamin D deficiency is extremely common in India, including in Uttarakhand, where sun exposure is often lower than people assume because of indoor work and covered clothing. It is also monitored in anyone taking calcium or vitamin D supplements at high dose, on long-term steroids, or after thyroid or parathyroid surgery.
