What is a Chikungunya PCR?

Chikungunya PCR detects the virus's own genetic material, making it the most specific confirmation available and the test of choice early in the illness. Its window is narrow and defined by the biology: virus circulates in the blood for roughly the first five to seven days of fever, after which it is cleared and antibodies take over as the way to detect infection. Chikungunya is spread by the same Aedes mosquitoes that carry dengue, breeding in small collections of clean standing water and biting mostly by day, so the two circulate together through the monsoon in Dehradun and the Doon valley and are frequently indistinguishable at the bedside. The feature that most separates them clinically is joint pain: chikungunya produces severe, often symmetrical arthritis of the hands, wrists, ankles and feet that can be genuinely disabling and, in a substantial minority, persists for months after the fever has gone. Dengue, by contrast, more characteristically lowers the platelet count and carries the risk of plasma leak. Knowing which one you have changes what your doctor watches for, even though neither has a specific antiviral treatment.

Why take this test?

Chikungunya PCR is advised in the first few days of a fever with severe joint pain, particularly during and after the monsoon, and especially where distinguishing it from dengue matters for monitoring. It is indicated when antibody testing would be too early to be informative, in severe or atypical illness, in pregnancy, and in outbreak surveillance. For most patients presenting later than about a week, an IgM antibody test is the appropriate choice instead, and testing for dengue in parallel is usually sensible since the two coexist. Whatever the result, treatment is supportive: rest, fluids and paracetamol for pain, avoiding ibuprofen and aspirin until dengue has been excluded because of bleeding risk. Seek medical care rather than waiting for a report if you develop persistent vomiting, severe abdominal pain, bleeding, breathlessness, drowsiness or a sudden drop in temperature with cold clammy skin.
