What is a TSH (Ultrasensitive) CMIA Serum?
TSH is made by the pituitary gland, not the thyroid, and understanding that inversion explains why it is the most useful single thyroid test. The pituitary continuously samples how much thyroid hormone is in circulation and adjusts its TSH signal to compensate. If the thyroid is underperforming, the pituitary shouts louder and TSH rises; if the thyroid is overproducing, the pituitary goes quiet and TSH falls. So the number moves in the opposite direction to the disease, which is the commonest source of confusion when people read their own reports: a high TSH means an underactive thyroid. The relationship is also logarithmic, meaning a small change in thyroid hormone produces a large swing in TSH, which is precisely what makes it so sensitive. It shifts before T3 and T4 leave their normal ranges and often before any symptom appears, and that early state, a raised TSH with normal hormones, is called subclinical hypothyroidism. The one situation where TSH misleads is a pituitary problem, since a failing pituitary cannot generate the appropriate signal, which is why free T4 is added when that is suspected.
Why take this test?
TSH is the standard first test for any suspicion of thyroid disease, and for most people it is the only thyroid test needed. It is advised for persistent tiredness, weight gain or loss without a change in diet, hair fall, dry skin, feeling unusually cold or unusually hot, constipation, palpitations, tremor, anxiety, low mood, poor concentration, irregular periods or difficulty conceiving. It is checked in pregnancy and in women planning one, because untreated hypothyroidism carries real risk to both mother and baby, and it is the number used to titrate a thyroxine dose, usually rechecked six to eight weeks after any change. Thyroid disorders are notably common across the sub-Himalayan belt that includes Uttarakhand, and they affect women several times more often than men, so vague long-running symptoms in this region are worth a TSH rather than dismissal.

What does a high TSH mean?
A high TSH usually suggests an underactive thyroid (hypothyroidism). Your doctor may add T3/T4 tests to confirm.
Why does a high TSH mean an underactive thyroid?
Because TSH is the pituitary's instruction, not the thyroid's output. When the thyroid underperforms, the pituitary raises TSH to push it harder, so the number climbs while the gland is doing less. It moves opposite to the disease, which is why the report can read backwards at first glance.
Do I need to fast, and does timing matter?
Fasting is not required. Timing matters a little, since TSH runs slightly higher in the early morning and dips later in the day. For consistency, especially when monitoring a dose, try to give the sample at roughly the same time each visit, ideally in the morning.
Should I take my thyroxine before the test?
Give the sample before your morning dose, then take the tablet afterwards. TSH itself is not much affected within a day, but keeping the routine consistent makes results comparable, and it matters more if free T4 is being measured at the same time. Never skip or alter a dose without medical advice.
My TSH is slightly high but I feel fine. Does it need treatment?
Not always. A mildly raised TSH with normal T4, called subclinical hypothyroidism, is common and is often simply rechecked after a few weeks, since transient illness and laboratory variation both cause it. Treatment depends on how high it is, whether you have symptoms, whether thyroid antibodies are present, and whether you are pregnant or planning to be.
