What is a Cytomegalovirus IgG (CMV)?

Cytomegalovirus is a herpes-family virus that is extremely common, particularly in India, where the great majority of adults have been infected, usually in childhood and usually without any illness worth remembering. Like other herpes viruses it is never cleared: after the first infection it persists lifelong in a latent state and can reactivate. IgG antibodies appear after that first infection and remain for life, so a positive result indicates past exposure and, in a healthy person, a reasonable degree of protection against the consequences of a fresh infection. It is used to interpret a positive CMV IgM by establishing whether prior immunity existed, which changes the meaning entirely. It is essential in transplant assessment, where the combination of donor and recipient status determines the risk of severe disease and the preventive strategy, and in people with HIV or on strong immunosuppression. It is not a useful routine test in a healthy non-pregnant adult, where a positive result, which is the norm, means nothing actionable.
