What is a Dengue PCR?

Dengue PCR detects the virus's own genetic material rather than a protein it sheds or the antibodies you make against it, which makes it the most specific dengue test available and the closest thing to a definitive answer. It works in the early viraemic phase, roughly the first five days of fever, when virus is circulating in the blood, and it is generally more sensitive than the NS1 antigen test during that window. Its distinctive additional capability is serotyping: it can identify which of the four dengue serotypes is responsible. That has limited relevance to how an individual patient is treated but real value where a second infection is suspected, since severe dengue is associated with sequential infection by a different serotype, and considerable value for public health surveillance during an outbreak. Its limitations are the mirror of its strengths. It becomes unreliable after about day five, once the virus has been cleared from blood even though the patient may be entering the most dangerous phase of illness, and it costs substantially more than an NS1 or antibody card while rarely changing what the treating doctor actually does.

Why take this test?

Dengue PCR is advised where confirmation genuinely matters and the patient presents early: in the first few days of fever when other tests have been equivocal, in a severe or atypical illness where the diagnosis is uncertain, in pregnancy or in an immunosuppressed patient where the stakes of a wrong diagnosis are higher, and where serotype information is wanted, whether for a suspected second infection or for outbreak surveillance. For the great majority of febrile patients in Dehradun during the monsoon, the combined NS1 with IgM and IgG card, alongside a Complete Blood Count to track platelets, answers the clinical question at a fraction of the cost. Choose this test on a doctor's advice rather than by default. Regardless of which test is used, the warning signs remain the same, and severe abdominal pain, persistent vomiting, bleeding, breathlessness or a sudden temperature drop with cold clammy skin mean hospital immediately, not another test.
