What is a E2 Estradiol?

Estradiol is the most potent of the oestrogens and, in women of reproductive age, is produced chiefly by the developing follicles in the ovary. Its level therefore reflects ovarian activity directly, and it changes markedly across the menstrual cycle: low at the start, rising steeply as a follicle matures, peaking just before ovulation, then settling into a secondary rise in the second half. That variation makes cycle timing essential to interpretation, and a result reported without the cycle day is close to meaningless. The measurement is used in two opposite directions. A low estradiol with a high FSH indicates the ovaries are no longer responding, the pattern of menopause or of premature ovarian insufficiency in a younger woman. A high estradiol is monitored during fertility treatment, where it tracks how many follicles are developing and is one of the signals used to time egg collection and to watch for ovarian hyperstimulation. In men, a raised estradiol is relevant to breast tissue enlargement and to fertility problems.

Why take this test?

Estradiol is advised in the assessment of irregular, infrequent or absent periods, of difficulty conceiving, and of suspected menopause or premature ovarian insufficiency, where it is measured with FSH and LH on days two to three of the cycle to assess ovarian reserve and function. It is used to monitor ovarian response during fertility treatment, and to assess the adequacy of hormone replacement therapy. It is also relevant in the investigation of delayed or precocious puberty, and in men with gynaecomastia, infertility or reduced libido where an oestrogen excess is suspected. Book the sample for the cycle day your doctor specifies, usually day two or three unless told otherwise, and tell the laboratory the date your last period started along with any hormonal medication you take, since the contraceptive pill and hormone therapy both alter the result substantially.
