What is a Magnesium?

Magnesium is a cofactor for hundreds of enzyme reactions and is essential to nerve conduction, muscle contraction, heart rhythm and bone structure. Its measurement carries one important caveat that shapes interpretation: only about one percent of the body's magnesium is in blood, the rest being inside cells and in bone, so the serum level is an imperfect reflection of total body stores and someone can be genuinely depleted while the blood level still reads normal. Despite that, a low serum magnesium is clinically significant when found. It causes muscle cramps and twitching, tremor, weakness, numbness, poor appetite, nausea and, at lower levels, disturbances of heart rhythm and seizures. It has a further consequence that is easy to miss: magnesium deficiency causes a low potassium and a low calcium that will not correct until the magnesium itself is replaced, so persistently low potassium despite supplementation is a classic clue. A magnesium test is advised for unexplained muscle cramps, twitching or tremor, persistent weakness, numbness or tingling, palpitations or a documented rhythm disturbance, and for seizures without an obvious cause. It is specifically indicated where potassium or calcium remain low despite replacement, because magnesium deficiency is a frequent and frequently overlooked reason for that. It is checked in people on long-term diuretics or proton pump inhibitors, in chronic alcohol use, in poorly controlled diabetes, in malabsorption or after bowel surgery, and in prolonged diarrhoea or vomiting. It is also part of bone assessment alongside calcium, phosphorus and vitamin D. It is usually ordered within an electrolyte or metabolic panel rather than alone, since magnesium abnormalities rarely occur in isolation and the surrounding values are what explain them.
