What is a Maternal Screen (Quadruple marker for 2nd trimester)?

The Quadruple Marker test is a second-trimester screening blood test, done at 15-20 weeks, that measures four markers: AFP, hCG, unconjugated estriol and inhibin A. Adding inhibin A to the triple test improves detection of Down syndrome. Combined with your age and gestational age, it estimates the risk of chromosomal conditions and open neural-tube defects.

What it measures

AFP (Alpha-fetoprotein)

High levels can indicate an open neural-tube defect; low levels shift chromosomal risk.

hCG

Pregnancy hormone contributing to the risk calculation.

Estriol (uE3)

A placental hormone; low levels can raise risk.

Inhibin A

The fourth marker; improves detection of Down syndrome over the triple test.

Why take this test?

It is used for second-trimester screening, especially when the first-trimester screen was missed, and gives slightly better detection than the triple marker.

  • Screening between 15 and 20 weeks of pregnancy
  • Missed or unavailable first-trimester screening
  • Maternal age or family history of chromosomal conditions
  • To additionally screen for neural-tube defects
