What is a OGCT (Pregnancy 1 Sample)?
This is the single-sample glucose challenge test, and its distinguishing feature is that no fasting is required. You drink a measured 50 gram glucose solution at any time of day, whether or not you have eaten, and one blood sample is taken exactly one hour later. That convenience is the entire rationale: it allows screening to happen opportunistically at an antenatal visit rather than requiring a woman to return fasting on another day, which in practice substantially improves how many women actually get screened. It is a screening test only. A raised value does not diagnose gestational diabetes; it identifies the pregnancies that should go on to a full Oral Glucose Tolerance Test, with fasting and three timed samples, which is what makes the diagnosis. Most women who screen positive have a normal tolerance test. Indian antenatal practice varies: some services use this two-step approach, while others use a single-step tolerance test for everyone, so which route you take depends on your obstetrician rather than on anything about you.
Why take this test?
This test is offered as part of routine antenatal care, usually around 24 to 28 weeks when placental hormones peak and gestational diabetes typically appears, and earlier where risk is higher: a previous pregnancy affected by gestational diabetes, a parent or sibling with diabetes, obesity, polycystic ovary syndrome, or a previous large baby. South Asian women carry a higher background risk than most other populations, which is why Indian practice screens broadly rather than only on risk factors. The reason it matters is that gestational diabetes is silent, and untreated it raises the risk of an oversized baby, a difficult or operative delivery, low blood sugar in the newborn and pre-eclampsia. Identified, it is very often managed with diet and monitoring alone. If your result is raised, it is a prompt for the confirmatory tolerance test, not a diagnosis, and hearing that clearly is worth more than anything else this page can tell you.
- Routine antenatal screening around 24 to 28 weeks
- Where a non-fasting screening option is more practical
- Gestational diabetes in a previous pregnancy
- A parent or sibling with diabetes, or obesity or PCOS
- A previous large baby
- As the first step of a two-step screening approach

Do I really not need to fast?
Correct, and that is the point of this version. You can have it at any time of day whether or not you have eaten, which makes opportunistic screening at an antenatal visit possible and means far more women actually get screened.
My result was high. Do I have gestational diabetes?
Not necessarily, and this is the commonest misunderstanding. A raised screening result means the confirmatory Oral Glucose Tolerance Test should follow. Most women who screen positive have a normal tolerance test.
What is the difference between this and the OGTT?
This screens with one non-fasting sample after a 50 gram drink. The tolerance test diagnoses, using an overnight fast, a 75 gram load and three timed samples over two hours. Screening sorts; diagnosis decides.
What if I vomit the glucose drink?
Tell the phlebotomist immediately. The test has to be abandoned and rescheduled, because the amount absorbed is no longer known. Brief nausea without vomiting is common and the test can continue.
Should I sit still during the hour?
Yes, stay at rest and take only water. Walking about, eating, smoking or a sugary drink during the hour will alter how quickly glucose clears and make the result unreliable.
