What is a Phosphorus?

Phosphorus is stored overwhelmingly in bone alongside calcium, with a small circulating fraction that is essential to energy metabolism, cell membranes and DNA. Its blood level is controlled by the kidneys under the influence of parathyroid hormone and vitamin D, and it moves in a broadly inverse relationship with calcium, which is why the two are almost always interpreted together. That relationship is what makes phosphorus informative. A high phosphorus with a low calcium is characteristic of chronic kidney disease, where the failing kidney cannot excrete phosphate and the resulting disturbance drives bone disease over time. A low phosphorus with a high calcium suggests an overactive parathyroid gland, which pushes phosphate out through the kidney. A low phosphorus alone occurs in vitamin D deficiency, malabsorption, alcohol excess, refeeding after prolonged starvation, and with prolonged use of antacids that bind phosphate. Severe depletion causes muscle weakness, including of the respiratory muscles, confusion and impaired heart function, so it is not a trivial abnormality despite being less familiar than calcium.

It is essential monitoring in chronic kidney disease, where phosphate retention drives bone and vascular complications, and it is checked in suspected parathyroid disorders. It is relevant in malnutrition, malabsorption, chronic alcohol use, and specifically during refeeding after prolonged starvation or severe illness, where a rapid fall in phosphate is a recognised and dangerous complication requiring anticipation rather than reaction. It is also measured in children with rickets and growth abnormalities. It is rarely useful alone; the pairing with calcium, and often with parathyroid hormone and vitamin D, is what allows the pattern to be read.
