What is a Potassium?
Potassium is the principal ion inside cells, and the small fraction circulating in blood is held within a strikingly narrow range because heart and muscle cells depend on that gradient to generate electrical activity. This is the one routine electrolyte result that can be an emergency on its own. A high potassium can cause dangerous heart rhythm disturbances with very few warning symptoms, sometimes progressing to cardiac arrest, and a very low potassium does the same from the other direction while also causing profound muscle weakness. Causes of a high value include kidney impairment, which is the commonest, medicines such as ACE inhibitors, ARBs and potassium-sparing diuretics, and tissue breakdown. Low values come from vomiting, diarrhoea, diuretics and, importantly, magnesium deficiency, which causes potassium loss that will not correct until the magnesium is replaced.
