What is a Potassium?

Potassium is the principal ion inside cells, and the small fraction circulating in blood is held within a strikingly narrow range because heart and muscle cells depend on that gradient to generate electrical activity. This is the one routine electrolyte result that can be an emergency on its own. A high potassium can cause dangerous heart rhythm disturbances with very few warning symptoms, sometimes progressing to cardiac arrest, and a very low potassium does the same from the other direction while also causing profound muscle weakness. Causes of a high value include kidney impairment, which is the commonest, medicines such as ACE inhibitors, ARBs and potassium-sparing diuretics, and tissue breakdown. Low values come from vomiting, diarrhoea, diuretics and, importantly, magnesium deficiency, which causes potassium loss that will not correct until the magnesium is replaced. One artefact matters greatly: if red cells rupture during collection or the sample sits too long, potassium leaks out and reads falsely high, which is why an unexpected result in someone well is usually repeated before anyone acts.

Why take this test?

Potassium is advised for muscle weakness, cramps, palpitations, an irregular pulse or fatigue, and it is essential monitoring for anyone with chronic kidney disease, heart failure or diabetes, and for anyone taking diuretics, ACE inhibitors, ARBs, spironolactone or potassium supplements. It is checked in prolonged vomiting or diarrhoea, in dehydration, and after any acute illness. It is standard before surgery and in any acute assessment. It is measured within an electrolyte panel with sodium and chloride and usually with creatinine, since kidney function is the main determinant of potassium handling. If your result is markedly abnormal, contact your doctor promptly rather than waiting for a routine appointment, and never take potassium supplements or salt substitutes, many of which are potassium-based, without medical advice, particularly if you have kidney disease or take any of the medicines listed above.
