What is a Procalcitonin?

Procalcitonin is a precursor of the hormone calcitonin, and in health almost none circulates. During a serious bacterial infection, bacterial toxins and inflammatory signals cause tissues throughout the body to release it, and levels can rise several thousandfold within hours. What makes it more useful than older inflammatory markers is that viral infections tend to suppress its production, so procalcitonin discriminates bacterial from viral infection considerably better than CRP or ESR do. It also rises and falls quickly, which allows it to track whether treatment is working. Its main clinical value is therefore twofold: supporting a decision to start antibiotics when infection is uncertain, and, just as importantly, supporting a decision to stop them, since a falling procalcitonin gives objective grounds for shortening a course. It is not infallible. Major surgery, severe trauma, burns, cardiac arrest and some cancers raise it without bacterial infection, and localised infections such as an abscess or an early urinary infection may not raise it much at all, so it is never read in isolation.

Why take this test?

Procalcitonin is advised where distinguishing bacterial from viral infection genuinely changes management and the usual markers have not settled it: an unwell patient with fever of unclear cause, suspected sepsis, suspected pneumonia where the decision to give antibiotics is finely balanced, or a patient deteriorating without an obvious source. It is used in intensive care and inpatient settings to guide antibiotic duration, where serial measurements showing a falling level support stopping treatment earlier and so reduce unnecessary antibiotic exposure. It is not a routine outpatient test and does not belong in a general health check. If you are unwell enough for procalcitonin to be relevant, you need clinical assessment rather than a blood test ordered independently, and the result must be interpreted by the doctor treating you alongside your examination, blood counts, cultures and imaging.
