What is a RA Factor for Quantitative?

Rheumatoid Factor is an antibody that, unusually, targets another antibody: it binds to the body's own immunoglobulin G. Its association with rheumatoid arthritis is long established, but its limits need stating clearly because this is one of the most misread tests in general practice. It is neither specific nor sensitive enough to diagnose anything by itself. Roughly a fifth to a third of people with genuine rheumatoid arthritis test negative, a state called seronegative disease, and it is often negative in the early months when treatment would do the most good. Conversely it is positive in a meaningful proportion of entirely healthy people, and the proportion rises with age, so a mildly positive result in someone over 65 with no joint symptoms usually means nothing at all. It is also raised in other conditions altogether: Sjogren's syndrome, chronic hepatitis C, tuberculosis, and long-standing infections. This is why modern practice pairs it with anti-CCP, which is far more specific for rheumatoid arthritis, and reads both alongside CRP or ESR and, above all, the clinical picture.

Why take this test?

An RA Factor test is advised for joint pain with swelling, particularly when it affects the small joints of the hands, wrists and feet, occurs on both sides of the body, and comes with morning stiffness lasting more than about half an hour. That pattern, stiffness that eases as the day goes on, is what distinguishes inflammatory arthritis from ordinary wear-and-tear osteoarthritis, where stiffness follows activity instead. It is also used in the assessment of other suspected autoimmune conditions, and a high titre carries some prognostic weight, since strongly positive disease tends to be more aggressive and may prompt earlier, firmer treatment. It should be ordered alongside anti-CCP and an inflammatory marker rather than alone. Persistent joint swelling deserves a rheumatologist rather than a blood test in isolation, because early treatment materially changes long-term joint damage.
