What is a SGPT / ALT?
SGPT, also written as ALT, is an enzyme concentrated almost entirely inside liver cells, where it helps convert protein into energy. Very little of it belongs in the bloodstream. When liver cells are inflamed, injured or dying, they leak SGPT out and the blood level climbs, which is why it is the single most sensitive routine marker of liver health and the first number most doctors look at on a liver function test. Because SGPT is so liver-specific, a raised value points at the liver far more directly than SGOT does, since SGOT also comes from heart and skeletal muscle. The degree of rise carries information too: modest elevations are typical of fatty liver and alcohol, while very high values suggest acute viral hepatitis or a drug reaction. A normal SGPT does not completely exclude liver disease, because a scarred, late-stage liver may have few cells left to leak.
