What is a Urea?

Urea is the waste your body makes when it breaks protein down for energy. The liver manufactures it, the bloodstream carries it, and the kidneys filter it out into urine. Measuring how much is left circulating gives a picture of how well that clearance is working. Urea is more sensitive than creatinine to things other than the kidneys, and that is both its weakness and its use. It climbs with dehydration, with a very high-protein diet, with bleeding into the stomach or intestine, where blood is digested as protein, and with steroid treatment; it falls in advanced liver disease, where the liver can no longer make it, and in pregnancy. Because of that, urea is never read alone. Its real value is in the ratio: a urea that has risen much faster than creatinine points towards dehydration or a gut bleed, while the two rising together points at the kidneys themselves. Some reports label it BUN, blood urea nitrogen, which measures only the nitrogen portion and so reads roughly half the urea figure.

Why take this test?

A urea test is advised whenever kidney function needs checking: swelling of the ankles or face, passing less urine than usual, persistent tiredness, nausea with a poor appetite, or an unpleasant taste in the mouth. It is part of the standard annual review for anyone living with diabetes or high blood pressure, the two leading causes of chronic kidney disease in India. Doctors also use it to assess someone who is clearly dehydrated from vomiting, diarrhoea or heat, to check on a patient with a suspected gastrointestinal bleed, and to monitor established kidney disease over time. In Dehradun and the surrounding hill districts, dehydration during the summer months is a common reason for a mildly raised urea with a normal creatinine. It is almost always ordered as part of a kidney function panel with creatinine, uric acid and electrolytes.
