What is a Urea BUN?

Blood urea nitrogen measures the nitrogen component of urea, the waste product the liver makes when protein is broken down and the kidneys clear into urine. It is the same underlying substance as blood urea reported a different way: because it counts only the nitrogen atoms, a BUN value reads roughly half the corresponding blood urea figure, which is a frequent source of confusion when comparing reports from different laboratories. Its clinical usefulness lies in being sensitive to more than the kidneys. It rises with dehydration, with a high protein intake, with bleeding into the stomach or intestine where blood is digested as protein, with steroid treatment and in heart failure where kidney perfusion falls; it falls in advanced liver disease, where the liver can no longer manufacture urea, and in pregnancy. That breadth means BUN alone is a poor measure of kidney function, and its real value emerges in the ratio to creatinine: a BUN that has risen disproportionately points towards dehydration or a gut bleed, while both rising together points at the kidneys.

BUN is advised as part of kidney function assessment, so most people encounter it within a panel rather than alone. It is indicated for swelling of the ankles or face, passing less urine than usual, persistent tiredness, nausea with poor appetite, or an unpleasant taste in the mouth. It forms part of the annual review for anyone with diabetes or high blood pressure, the leading causes of chronic kidney disease in India, and for anyone on long-term painkillers or medicines cleared by the kidneys. It is used to assess dehydration in vomiting, diarrhoea or heat exhaustion, which matter through the Dehradun summer, and to investigate a suspected gastrointestinal bleed, where a disproportionately raised BUN with a normal creatinine is a recognised clue.
