What is a Valproic Acid?

Valproate is used for epilepsy, bipolar disorder and migraine prevention, and it belongs to the small group of medicines where measuring the blood level genuinely helps. That is because it has a narrow therapeutic window: too little and seizures or mood episodes continue, too much and toxicity appears as drowsiness, tremor, unsteadiness, nausea and confusion. It also has awkward pharmacology. It binds heavily to albumin and that binding saturates, so the relationship between dose and free active drug is not linear, and in someone with a low albumin the total measured level can look reassuring while the active fraction is higher than it appears. Levels are also affected by other medicines, notably some antibiotics that lower them sharply. Therapeutic drug monitoring answers specific questions: whether an inadequate response reflects an inadequate level or a drug that simply is not working, whether new symptoms are toxicity, and whether a patient is taking the medicine as prescribed. The level is always interpreted alongside how the patient actually is, not treated as a target in itself.

Why take this test?

A valproate level is advised when seizures or mood symptoms continue despite treatment, to establish whether the level is adequate before changing to another drug; when symptoms suggest toxicity, particularly drowsiness, unsteadiness, tremor, vomiting or confusion; after any dose change, to confirm the new steady state; when another medicine has been started or stopped that might interact; and in pregnancy, liver disease or kidney impairment where handling of the drug alters. It is also used where adherence is uncertain. Timing is essential to a meaningful result: the sample should normally be a trough, taken just before the next dose, so that levels are comparable between visits. Never adjust your own dose on a level, and never stop valproate abruptly, since sudden withdrawal can precipitate seizures. Valproate also carries significant risks in pregnancy, which is a conversation to have with your doctor in advance rather than at the point of testing.
