What is a Bleeding Time (BT)?

The bleeding time measures how long a small standardised cut on the forearm continues to bleed, assessing in principle how quickly platelets plug a damaged vessel. It is one of the oldest bedside tests of haemostasis and it needs describing honestly, because it is still ordered routinely in India as part of pre-operative screening while most international guidance has abandoned it. The reasons are well established. The result depends heavily on how the test is performed, on the depth of the incision and on the operator, giving poor reproducibility between users. More importantly, it has been shown repeatedly not to predict surgical bleeding, which is the very purpose for which it is usually requested, so a normal bleeding time gives false reassurance and an abnormal one causes delay without adding useful information. It can be prolonged in genuine platelet disorders, in von Willebrand disease, in severe thrombocytopenia and by aspirin, but each of those is better identified by a platelet count, a coagulation profile and, where indicated, specific platelet function testing or von Willebrand studies.

Why take this test?

In current practice, bleeding time is most often performed because it is included in a pre-operative package rather than because a specific question requires it. Where a bleeding disorder is genuinely suspected, the far more informative approach is a careful bleeding history, a full blood count with platelet count, a prothrombin time and APTT, and where indicated von Willebrand factor studies or formal platelet function testing. The bleeding history matters most of all: easy bruising, prolonged bleeding after dental extraction or minor cuts, frequent nosebleeds, heavy menstrual periods, or a family history of bleeding are what should prompt investigation, and any of those deserves proper assessment regardless of a normal bleeding time. If your doctor has ordered it as part of a standard panel that is reasonable, but do not treat a normal result as clearance to proceed if you have a genuine bleeding history to disclose.
