What is a Differential Leukocyte Count (DLC)?

The Differential Leukocyte Count breaks your white blood cells into their five types and reports each as a percentage of the total. Those five do different jobs, and that is what makes the breakdown useful. Neutrophils are the first responders to bacterial infection and typically dominate. Lymphocytes handle viruses and long-term immune memory. Monocytes clear debris and take over in longer-running infections such as tuberculosis. Eosinophils rise with allergy and parasitic worm infestation. Basophils, the rarest, are involved in allergic reactions. A total white-cell count tells you only that the immune system is busy; the differential tells you what kind of fight it is having. A neutrophil-heavy pattern points towards bacteria, a lymphocyte-heavy one towards a virus, and a high eosinophil share towards allergy or worms, all of which are common in India. Because these are percentages that must sum to 100, one cell type rising automatically pushes the others down, so the absolute counts are often more meaningful than the percentages alone.

Why take this test?

A DLC is advised for fever of unclear cause, when an infection is suspected but its type is not obvious, for recurrent or lingering illness, and for allergic symptoms such as persistent sneezing, wheezing, itching or a rash. It is a standard next step whenever a Complete Blood Count reports a white-cell total that is too high or too low, because the total alone does not say which cell line is responsible. Doctors also use it to look for worm infestation in someone with unexplained anaemia or abdominal symptoms, to follow the course of a known infection, and to keep watch on patients taking medicines that suppress the bone marrow. In practice it is almost never ordered on its own: it comes as part of a CBC, where the differential is read together with haemoglobin, platelets and the red-cell indices.
