What is a Erythrocyte Sedimentation Rate (ESR)?
ESR is one of the oldest tests still in routine use, and it works on a simple physical principle. Blood is left standing in a narrow tube for an hour and the distance the red cells fall is measured. Inflammation raises certain proteins in plasma, particularly fibrinogen, which make red cells stick together in stacks; heavier stacks sink faster, so a raised ESR indicates that those proteins are elevated. Because it measures a downstream consequence rather than inflammation directly, ESR is slow. It takes days to rise after a problem begins and weeks to fall after it resolves, which is the opposite of CRP's hours. That sluggishness is a weakness for acute illness and a strength for chronic disease, where ESR tracks the long arc of activity. It is also easily influenced by things that have nothing to do with inflammation: anaemia and pregnancy raise it, age raises it, and polycythaemia lowers it. For these reasons ESR is never diagnostic on its own and is usually read with CRP and a Complete Blood Count.
