What is a Erythrocyte Sedimentation Rate (ESR)?

ESR is one of the oldest tests still in routine use, and it works on a simple physical principle. Blood is left standing in a narrow tube for an hour and the distance the red cells fall is measured. Inflammation raises certain proteins in plasma, particularly fibrinogen, which make red cells stick together in stacks; heavier stacks sink faster, so a raised ESR indicates that those proteins are elevated. Because it measures a downstream consequence rather than inflammation directly, ESR is slow. It takes days to rise after a problem begins and weeks to fall after it resolves, which is the opposite of CRP's hours. That sluggishness is a weakness for acute illness and a strength for chronic disease, where ESR tracks the long arc of activity. It is also easily influenced by things that have nothing to do with inflammation: anaemia and pregnancy raise it, age raises it, and polycythaemia lowers it. For these reasons ESR is never diagnostic on its own and is usually read with CRP and a Complete Blood Count.

Why take this test?

An ESR is advised for unexplained fever, persistent joint pain or morning stiffness, unexplained weight loss, and where a chronic inflammatory or autoimmune condition such as rheumatoid arthritis is suspected. In India it retains a specific role in tuberculosis, where it is often markedly raised and is used alongside other investigations, and it is used to follow treatment response over weeks in conditions where a single week's change means little. It is a standard component of a Complete Blood Count with ESR, which is one of the most frequently ordered panels in Indian general practice. Its value is greatest when paired with CRP, because the two move on different timescales: a high CRP with a normal ESR usually means something has just started, while a high ESR with a settling CRP suggests a process that is already resolving.
