What is a Cervical Screening-2 Biopsy?

A cervical biopsy takes a small piece of tissue from the cervix so that a pathologist can examine its full thickness, and it sits one step beyond screening in the diagnostic sequence. A Pap smear collects loose surface cells and can suggest that abnormal changes are present; an HPV test identifies the virus that causes them; but only a biopsy shows how far through the cervical lining those changes extend, which is what determines whether and how they are treated. The findings are graded as cervical intraepithelial neoplasia, CIN 1, 2 or 3, according to how much of the epithelial thickness is involved. That grading directly drives management: CIN 1 frequently resolves on its own and is usually observed with repeat testing, while CIN 2 and 3 are generally treated, most often by removing the affected zone with a LEEP or cone excision. The whole framework exists because cervical cancer develops slowly over ten to fifteen years through these stages, and treating them prevents the cancer entirely. A cervical biopsy is advised after an abnormal screening result: a Pap smear showing significant cell changes, persistent high-risk HPV infection, or a combination of the two. It is also indicated when the cervix looks abnormal to the naked eye during examination, and for symptoms that must not be attributed to screening findings alone, notably bleeding between periods, bleeding after intercourse, bleeding after menopause, or a persistent unusual discharge. Those symptoms need a gynaecologist promptly rather than a routine smear. The purpose of the biopsy is to establish exactly what is present so that treatment is neither withheld nor given unnecessarily, since over-treating low-grade changes has its own consequences for future pregnancies.
