What is a Histo Extra Large?

This is the most involved tier of histopathology, applied to major resection specimens: a radical bowel resection, a mastectomy with axillary clearance, a total gastrectomy, a nephrectomy or a lung lobectomy. The report it produces is not simply a diagnosis but the document on which the entire subsequent treatment plan rests. The pathologist works through a systematic protocol: orienting and measuring the specimen, describing the tumour's size and appearance, sampling it and the surrounding tissue in multiple blocks, examining how deeply it invades through the layers of the organ, checking every surgical margin for involvement, and dissecting out and examining each lymph node individually. From these come the pathological stage, the tumour grade, the presence of invasion into blood vessels or lymphatics, and, for many cancers, receptor or molecular status that determines which drugs will work. This is why the report takes a week or more, and longer where immunohistochemistry or molecular testing is needed. It is the most labour-intensive investigation in the laboratory and the one that most directly shapes what happens next.

Why take this test?

This examination follows major surgery and is not something ordered independently: every significant resection specimen goes for it as a matter of course. Its purpose is to convert an operation into a staged diagnosis and a treatment plan. The pathological stage established here is more reliable than the clinical stage estimated from pre-operative imaging, because it rests on direct examination rather than inference, and it is what determines whether chemotherapy, radiotherapy or hormonal treatment is recommended and for how long. Margin status determines whether further surgery is needed. Node status is among the strongest predictors of outcome. Because the report drives all of this, definitive treatment decisions generally wait for it, and your surgeon and oncologist will discuss it with you together.
