What is a PAPP-A?
PAPP-A is a protein produced by the placenta, and its level in the first trimester reflects how the placenta is developing. It is the second blood marker of the combined first-trimester screen, paired with free beta hCG and read alongside the nuchal translucency on ultrasound and the mother's age to produce a risk estimate for Down syndrome and certain other chromosomal conditions. In affected pregnancies PAPP-A tends to be lower than expected while free beta hCG tends to be higher, and it is the combination that gives the screen its discriminating power. PAPP-A carries a second, separate significance that is often overlooked: a low level, even when the chromosomal risk comes back low, is associated with placental underperformance later in pregnancy, and is linked to a higher chance of fetal growth restriction, pre-eclampsia and preterm birth.
