What is a General Blood Picture (GBP)?

A General Blood Picture is a pathologist examining your blood cells directly under a microscope, rather than a machine counting them. That distinction is the whole point. An automated Complete Blood Count is fast, precise and excellent at numbers, but it describes populations, not individuals: it reports that red cells average a certain size without noticing that some are teardrop-shaped, sickled or fragmented, and it counts white cells without recognising that some are immature forms that should never be in circulation. The smear catches exactly those things. It identifies the shape abnormalities that point to specific diagnoses, spots immature or blast cells that raise the question of a blood cancer, sees parasites such as malaria directly, notices platelet clumping that explains a falsely low automated platelet count, and assesses whether the marrow is responding appropriately to anaemia. It is qualitative rather than quantitative, and it depends on the skill of the person reading it, which is why it is a complement to the CBC rather than a replacement for it.

Why take this test?

A GBP is advised when a Complete Blood Count has produced an unexplained or flagged result and the numbers alone do not say why: an anaemia whose cause is unclear, an unexpectedly high or low white count, a very low platelet count, or an automated flag suggesting abnormal cells. It is used to investigate persistent unexplained fatigue, easy bruising or bleeding, recurrent infections, unexplained weight loss, or enlarged lymph nodes or spleen. It is a standard part of the work-up when a haemoglobin variant such as thalassaemia trait is suspected, and it is the test that finds malaria parasites in a febrile patient. Doctors also use it to follow known blood disorders and to check the marrow's response after treatment for anaemia. It is almost always ordered alongside a CBC rather than instead of it, since the numbers and the picture answer different halves of the same question.
