What is a General Blood Picture (GBP) + Peripheral Smear?

A peripheral smear is a thin film of blood spread across a glass slide, stained, and read by a pathologist cell by cell. It is one of the oldest investigations in medicine and remains one of the most informative, because it captures what automated counters cannot: the individual character of the cells. On the red cell side it identifies shapes that point to specific diagnoses, small pale cells suggesting iron deficiency, large oval cells suggesting B12 or folate deficiency, target cells and basophilic stippling raising thalassaemia trait, fragmented cells indicating that red cells are being physically damaged in the circulation, and sickled forms in sickle cell disease. On the white cell side it distinguishes a reactive response to infection from immature blast cells that suggest leukaemia, and it recognises the atypical lymphocytes of viral illness. Why take this test?

A peripheral smear is advised when a Complete Blood Count has flagged something the numbers cannot explain, when anaemia needs classifying beyond what the indices show, or when a blood disorder is suspected. It is indicated for unexplained fatigue with an abnormal count, easy bruising or bleeding, recurrent or unusual infections, unexplained weight loss, bone pain, or enlarged lymph nodes, liver or spleen. It is the definitive test for malaria in a febrile patient in Dehradun through the monsoon, and it is standard in the work-up for thalassaemia trait, which is common enough in India that it should be considered in any persistent microcytic anaemia that does not respond to iron.
