What is a HB + TLC?
This is the most basic blood panel, reporting haemoglobin and the total leukocyte count. Haemoglobin establishes whether anaemia is present and how severe it is; the total white count establishes whether the immune system is mobilised, suppressed, or unremarkable. Between them they answer the two questions that come up most often in general practice, and they do so at the lowest cost of any blood panel. The limitations are correspondingly broad and worth being explicit about. Without the differential count you know the white cells are raised but not whether neutrophils, lymphocytes or eosinophils are responsible, which is what points towards bacterial, viral or allergic causes. Without platelets you have no information on bleeding risk or on dengue. Without the red cell indices you cannot classify an anaemia. In practice this means the panel frequently identifies that something is wrong without indicating what, and a second test follows. It is best regarded as a screen for a specific narrow question rather than as an assessment of an unwell patient.

This panel is advised where the question is narrow and already framed: confirming anaemia in someone being treated for it, a quick check of whether an infection is producing a white cell response, or a basic screen where cost is the binding constraint. It is used in follow-up where the fuller picture is already established and only these two values need tracking.
