What is a HbsAg?

HBsAg is a protein from the outer coat of the hepatitis B virus, and finding it in blood means the virus is currently present. It is the first marker to appear after infection, usually within one to ten weeks of exposure and often before any symptoms, which is what makes it the standard front-line screen. Hepatitis B spreads through blood and body fluids: unsterile needles or instruments, transfusion, unprotected sex, and from mother to baby at birth, which remains a major route in India. The reason it matters is that the infection is frequently silent. Many people carry it for years with no symptoms while the virus slowly inflames the liver, and a proportion go on to cirrhosis or liver cancer. If HBsAg is still detectable after six months, the infection is considered chronic. A single HBsAg result cannot distinguish a recent infection from a long-standing one, nor tell you how active the virus is, so a reactive result always leads to further testing rather than to a conclusion. Importantly, hepatitis B is vaccine-preventable, and someone protected by vaccination tests HBsAg negative.

Why take this test?

An HBsAg test is advised before surgery, dialysis or any invasive procedure, as routine antenatal screening, and for blood and organ donors. It is indicated after possible exposure through a needlestick or sharps injury, an unsterile tattoo, piercing or dental procedure, sharing injecting equipment, or unprotected sex with a new or untested partner. It should be checked in anyone with unexplained abnormal liver enzymes, jaundice, or a family member with hepatitis B, since household and mother-to-child transmission are common. It is often run as part of a viral marker panel with hepatitis C and HIV. Testing matters because effective antiviral treatment exists for chronic hepatitis B, and because knowing your status protects the people around you: close contacts who are negative can be vaccinated, and a mother who knows her status can have measures taken at delivery that greatly reduce transmission to her baby.

FAQs: HbsAg in Dehradun

What does a reactive HBsAg mean?

A reactive (positive) HBsAg indicates a current Hepatitis B infection. Your doctor will order confirmatory and liver tests and guide you on next steps; it is not a diagnosis to act on alone.

Does a reactive result mean I will get liver disease?

No. Many people clear the infection, and among those who do not, outcomes vary widely and effective antiviral treatment exists. What determines risk is whether the infection becomes chronic and how active the virus is, which further tests establish. This is a result to follow up promptly, not to panic about.

I am vaccinated. Why is my HBsAg negative?

Because the vaccine gives you protective antibodies, not the surface antigen itself. A vaccinated person correctly tests HBsAg negative. If you want to confirm that vaccination worked, the test for that is anti-HBs, which measures the protective antibody, not this one.

How soon after exposure can HBsAg be detected?

Usually between one and ten weeks after exposure, so a test taken within days of a possible exposure can be negative even if infection has occurred. If exposure was significant, seek medical advice immediately rather than waiting to test, since preventive measures are time-critical, and expect the test to be repeated.

Should I test hepatitis C and HIV as well?

Often, yes. The three share transmission routes, so they are commonly ordered together as a viral marker panel, and that is standard before surgery and in pregnancy. Your doctor will advise based on the exposure and the reason for testing.
