What is a Widal Test?

The Widal test measures agglutinating antibodies against the O and H antigens of Salmonella typhi, and it is worth being candid about its shortcomings because it remains heavily over-relied upon in India. Antibodies take about a week to rise, so it is frequently negative in the first week of fever when patients present and when treatment decisions are made. In an endemic country a large share of the population carries background antibodies from previous exposure or vaccination, so a single raised titre may reflect old immunity rather than current illness. It cross-reacts with other Salmonella species, with malaria and dengue, and it can be suppressed by antibiotics already started. The classical way to use it properly is paired samples taken a week apart showing a fourfold rise in titre, which is genuinely meaningful but rarely done in practice because treatment cannot wait a week. A single Widal result should therefore never be the sole basis for a typhoid diagnosis, and blood culture, which grows the organism and provides antibiotic sensitivities, remains the reference standard.

Why take this test?

Widal is performed for a fever lasting more than about a week, particularly one climbing in a stepwise pattern with headache, abdominal discomfort, marked malaise, loss of appetite and either constipation or loose stools, during or after the monsoon in Dehradun and the Doon valley where typhoid spreads through contaminated food and water.
