What is a Viral Marker (3H)?

This panel screens for the three blood-borne viruses that share transmission routes and matter most in healthcare settings: HIV, hepatitis B via the surface antigen HBsAg, and hepatitis C via its antibody. They are grouped because the exposures are the same, unsterile needles and instruments, transfusion, sexual contact and mother-to-child transmission at birth, so someone at risk of one is usually at risk of the others. All three are screening tests, which has a specific meaning worth understanding. They are deliberately tuned to miss as few true infections as possible, and the price of that sensitivity is that a proportion of reactive results turn out not to be infection at all. No reactive result on this panel is a diagnosis. Each has its own confirmatory pathway: HIV requires further specific testing, a reactive HCV antibody requires HCV RNA to establish whether the virus is still present, and a reactive HBsAg requires additional hepatitis B markers and liver assessment. Each also has a window period after exposure during which the test can be negative despite infection.

Why take this test?

The panel is advised before surgery, dialysis, endoscopy and other invasive procedures, in pregnancy as part of antenatal screening, and for blood and organ donors, which is where most people encounter it. It is indicated after a possible exposure: a needlestick or sharps injury at work, an unsterile tattoo, piercing or dental procedure, sharing injecting equipment, or unprotected sex with a new or untested partner. It is also used to investigate unexplained abnormal liver enzymes or jaundice. The practical argument for testing is that all three are now treatable or manageable, hepatitis C is usually curable with oral tablets, hepatitis B is controllable with antivirals and preventable by vaccination, and HIV treated early allows a normal lifespan with no sexual transmission once the virus is undetectable. Testing is confidential, and Indian law requires informed consent and privacy for HIV testing in particular.
