What is a Lupus Anticoagulant Confirm?

The lupus anticoagulant is the most confusingly named test in this panel, and understanding the name explains the test. It was identified in lupus patients, hence the first word, and it prolongs clotting times in the test tube, hence the second. But in the body it does the opposite: it makes clotting more likely, not less. The laboratory effect is an artefact of how the antibody interferes with phospholipid-dependent clotting reactions, while the clinical effect is thrombosis. It is not measured as a concentration but demonstrated functionally, through a sequence: a screening test showing prolonged clotting, a mixing study confirming the prolongation is not due to a missing clotting factor, and a confirmatory step showing that adding excess phospholipid corrects it. Of the three antiphospholipid tests, this one correlates most strongly with actual thrombotic risk, so its presence carries particular weight. Why take this test?

Testing is advised for unexplained thrombosis, particularly in a young person, at an unusual site, or arterial without conventional risk factors, and for recurrent pregnancy loss meeting the defined criteria. It is indicated when a routine clotting test, most often the APTT, comes back unexpectedly prolonged in someone with no bleeding tendency, which is a classic and frequently overlooked clue. It is a core part of the lupus work-up. It is ordered alongside anticardiolipin and anti-beta-2 glycoprotein I, since the three identify overlapping but different patients. Timing matters more here than for the others: ideally test before starting anticoagulation, and tell the laboratory exactly what you take, because interpretation on warfarin or a direct oral anticoagulant requires specific handling and may need deferring. As with the whole panel, a positive result must be confirmed at least twelve weeks later before it means anything.
