What is a P/S for MP?

This is the direct search for malaria: a drop of blood is spread on a slide, stained, and examined under a microscope by a trained pathologist looking for the parasite inside red blood cells. It remains the reference standard against which other malaria tests are judged, and it does two things a rapid antigen card cannot. It identifies the species, which matters because Plasmodium falciparum can cause severe, life-threatening disease and requires different, more urgent treatment than Plasmodium vivax, the other common species in India. And it estimates the parasite load, the proportion of red cells infected, which is one of the measures used to judge severity and to follow whether treatment is working. Two limitations shape how it is used. Sensitivity depends heavily on when the sample is taken, because parasites are released into the bloodstream in waves that coincide with fever spikes, so a sample drawn between spikes can miss a genuine infection. And it depends on the microscopist's skill and on the parasite load being high enough to find, which is why a single negative smear never excludes malaria when suspicion is real.

Why take this test?

A malaria smear is advised for any fever with chills and rigors, sweating, headache and body ache, particularly during and after the monsoon in Dehradun and the Doon valley, or following travel to a malaria-endemic area. The textbook pattern of fever recurring in cycles is suggestive but is frequently absent, so it should not be waited for. Timing genuinely improves the yield: a sample taken during or immediately after a temperature spike is most likely to catch the parasite. Because a single negative is not conclusive, doctors routinely repeat the smear after 12 to 24 hours when the clinical picture still fits, and often pair it with a rapid antigen test and a Complete Blood Count, since falling platelets support the diagnosis and the counts help distinguish malaria from dengue. Seek medical care urgently, without waiting for any report, for drowsiness, confusion, repeated vomiting, breathlessness, very dark urine, reduced urine output or a fit.
