What is a Reticulocyte Count?

Reticulocytes are red blood cells released from the bone marrow within roughly the last day or two, still carrying remnants of the machinery that made them. Counting them measures production, which is the one thing a haemoglobin level cannot tell you, and it splits anaemia into two fundamentally different categories. If someone is anaemic and the reticulocyte count is high, the marrow is working hard and the problem lies in loss or destruction: bleeding, or haemolysis where red cells are being broken down early. If someone is anaemic and the reticulocyte count is low or normal, the marrow is not compensating and the problem is production: iron, vitamin B12 or folate deficiency, chronic disease, kidney disease where erythropoietin is lacking, or marrow failure. That single fork directs the entire subsequent investigation. The count is also the earliest signal that treatment is working, rising within about a week of starting iron or B12, well before the haemoglobin itself moves, which makes it a useful early confirmation that the diagnosis was correct.

Why take this test?

A reticulocyte count is advised whenever anaemia has been found and its mechanism needs establishing, which is most of the time. It is particularly useful where haemolysis is suspected, alongside bilirubin, LDH and a direct Coombs test, and where anaemia has failed to respond to treatment and the question is whether the marrow is even attempting to compensate. It is used to confirm early response to iron, vitamin B12 or folate replacement, typically checked around a week after starting, and to monitor marrow recovery after chemotherapy or a bone marrow transplant. It is also relevant in chronic kidney disease, where inadequate erythropoietin production suppresses it. It is drawn from the same sample as a Complete Blood Count and is usually ordered alongside one, since the count only means something when read against the haemoglobin and the red cell indices.
