What is a Malaria Ag?

The malaria antigen test is a rapid card that detects proteins released by the parasite rather than looking for the organism itself. Its practical advantages are speed and consistency: a result in about twenty minutes, no microscope, and no dependence on the skill of the person reading a slide, which is why it has become standard in fever assessment. Most cards distinguish Plasmodium falciparum from the non-falciparum species including vivax, and that distinction is clinically important, since falciparum can progress rapidly to severe malaria and needs urgent specific treatment, while vivax has a dormant liver stage requiring an additional drug to prevent relapse. Two limitations shape its use. Sensitivity falls at low parasite densities, so a negative card does not exclude malaria in a strongly suspicious case. And the falciparum antigen most cards use, HRP2, persists in blood for two to four weeks after successful treatment, so the test can remain positive in someone already cured, which means it cannot be used to judge whether treatment has worked. For that reason it complements the blood smear rather than replacing it.
