What is a Stool Routine?

A Stool Routine examines the sample three ways, and each layer answers something different. The physical examination records colour, consistency and the presence of visible mucus or blood, which alone can distinguish an inflammatory diarrhoea from a watery one. The chemical part tests for occult blood, which is blood present in quantities too small to see, and for reducing substances and pH, which indicate whether sugars are being malabsorbed, a common finding in children. The microscopic examination is where most of the value sits: it looks for pus cells, which point to invasive bacterial infection, red cells, undigested fat globules, and, importantly in India, the eggs, cysts and larvae of intestinal parasites such as Giardia, Entamoeba, roundworm and hookworm. Finding a parasite is definitive and changes treatment immediately. The test's main limitation is that it does not culture bacteria, so it can suggest bacterial infection through pus cells without naming the organism; where that matters, a stool culture is added. Parasites are also shed intermittently, which is why a single negative sample does not exclude infestation.

Why take this test?

A stool routine is advised for diarrhoea lasting more than a few days, blood or mucus in the stool, persistent abdominal pain or cramping, bloating and excessive gas, unexplained weight loss, or itching around the anus, which is a classic sign of worms. It is particularly worth doing where symptoms follow travel, a suspect meal, or drinking untreated water, all of which are ordinary routes to parasitic infection in this region. In children it is used to investigate chronic loose stools, poor weight gain and unexplained anaemia, since hookworm infestation is a recognised cause of iron deficiency. It is also part of investigating unexplained iron-deficiency anaemia in adults, where occult blood may be the first clue to bleeding somewhere in the gut that needs proper investigation rather than just an iron supplement. Doctors frequently ask for samples on three separate days when parasites are suspected, precisely because shedding is intermittent.
