What is a Urine Sugar?

The kidney normally reabsorbs all the glucose that passes through its filter, so healthy urine contains essentially none. Glucose appears only when blood sugar exceeds what the kidney can reclaim, a point called the renal threshold, which sits at roughly 180 mg/dL in most adults. That threshold is the key to understanding both the value and the limits of this test. A positive result means blood sugar has been high enough at some point to overflow, which is a meaningful finding. A negative result, however, proves very little: someone can have blood sugar in the diabetic range and still spill no glucose, because their level has not crossed the threshold. The threshold also varies between people. It falls in pregnancy, so a healthy pregnant woman may spill glucose at entirely normal blood levels, and it falls in a condition called renal glycosuria, where the kidney leaks glucose despite normal blood sugar. It rises in older adults and in long-standing kidney disease, masking genuinely high sugars. This is why urine sugar has been superseded by blood testing for diagnosis and survives mainly as a component of the urine routine.

As a standalone test it is advised where a quick, non-invasive check is wanted, and its most useful role today is as an incidental flag: glucose found unexpectedly on a routine urine test in someone not known to have diabetes is a genuine prompt to check blood sugar properly. It is also used in pregnancy, where persistent glycosuria may prompt earlier screening for gestational diabetes, though a single positive is common and not alarming. It should never be used to diagnose diabetes, to exclude it, or to monitor control in someone already diagnosed; fasting sugar, postprandial sugar and HbA1c do those jobs far better.
