Swelling in the legs and feet: what to check

Swelling in the legs, feet, or ankles (called oedema) often comes from harmless things like long standing or hot weather. But swelling that persists can be a sign that the kidneys, liver, or heart are not handling fluid and protein well. A few targeted tests help point to the cause, alongside a doctor's review.

Swelling in legs & feet, a LabONE health guide

When swelling is worth testing

  • Ankles or feet that stay swollen
  • Swelling that is worse by the evening
  • Puffiness around the eyes, especially in the morning
  • Foamy urine or a change in urination
  • A known kidney, liver, or heart condition
  • Swelling with breathlessness (see a doctor urgently)

What the tests look for

Kidney function (creatinine and a Kidney Function Test) with a urine routine check whether the kidneys are leaking protein or struggling to clear fluid. A Liver Function Test, including albumin, checks whether the liver is making enough of the protein that keeps fluid in the vessels. Heart-related causes need a doctor's clinical assessment.

When to see a doctor

Sudden, painful, one-sided, or breathless swelling needs urgent medical care and is not something to test for at home first. For persistent swelling on both sides, the kidney, liver, and urine panel above is a sensible first step to do alongside a doctor's review.

This guide is for general information and is not a diagnosis. Always discuss your symptoms and results with a doctor. LabONE includes a free doctor consultation with every report.

Frequently asked questions

What causes swollen feet and ankles?

Many things, from long standing and heat to medication side effects. But swelling that persists can involve the kidneys, liver, or heart, which is why a few blood and urine tests plus a doctor's review are worthwhile.

Which tests help for leg swelling?

Kidney function (creatinine/KFT), a urine routine to check for protein, and a liver function test are the usual first checks. Persistent or one-sided swelling should be seen by a doctor.
