What is a Fluid Routine Examination?

When fluid accumulates abnormally in the chest, abdomen, a joint or around the heart, analysing it is usually the fastest route to understanding why. The examination has three parts. Appearance alone is informative: clear straw-coloured fluid, cloudy pus, blood-stained fluid and milky fluid each point in different directions. The cell count and differential separate the possibilities further, with neutrophil predominance suggesting bacterial infection, lymphocyte predominance raising tuberculosis or malignancy, and red cells raising trauma or malignancy. The chemistry, principally protein, albumin, glucose and LDH, distinguishes the two fundamental categories: a transudate, which is fluid pushed out by pressure or low protein as in heart failure, cirrhosis or kidney disease, and an exudate, where the lining itself is inflamed or invaded as in infection, tuberculosis or cancer. That transudate-exudate distinction is the single most useful output of the test, because it splits the differential diagnosis in half and determines everything that follows. Cytology for malignant cells, culture and tuberculosis testing are typically sent from the same tap.
